Is Paypal the only gateway currently available for Membership Suite? And if so, do you do any custom programming for an additional fee?
Currently PayPal is the only gateway supported. But I am heavily in development of the next generation of my modules which will, among other things, provide support for more payment gateways, such as Authorize.net and Google Checkout. I am actually coding it now, as we speak, haha.

Any future plans to offer integration with Fastspring? They have very interesting tax management capability.

i need to add some more very simple payment options (pay by cheque, pay by bank transfer), that simply record the payment attempt in the database, to be filled later manually when payment is received.
The simplest way to do it would be create a new payment gateway for each option?

Yes, the best way is to create payment gateways for those types of payments. I have been meaning to create payment gateways that support that for quite some time, but haven't found the time yet ><

You can now manually add memberships, so you can accecpt payment any way you choose, and then manually upgrade the accounts to the membership plan that you choose.
For example, you could accept a check for X amount and then add the membership, and set for it to expire in a month, 2 months, etc.
