We have a number of orders with status Payment Pending in the orders list:
/admin/moneyscripts/orders
Example:
W8U6H3 [user blank] None Pending Payment Premium Subscriber $0.00
The only active gateway is PayPal Payments Standard, and other orders are going through fine through that (thanks!).
Question: Do these represent:
a) Abandoned carts?
b) User clicks Checkout, goes to PayPal, and abandons?
c) User clicks Checkout, goes to PayPal, pays, and fails to click back to the site ("Return to vendor")?

Pending payment orders are orders that went to PayPal in your case, and then were abandoned. If the user pays, the order status would never be pending payment, unless there is some problem with your IPN setup.
But if any of your other orders are marked as completed or active, then there is no problem with your IPN setup.
